Our clinician/psychotherapist, Dr. Selena LaMotte DSW, LCSW, is licensed by the State of Florida to assess, diagnose, and treat mental health disorders.
Our office provides Teletherapy/Video sessions ONLY for individuals 18+. We are currently only accepting Florida Blue, Optum, Sunshine, and UnitedHealth insurance at this time. We are sorry; unfortunately, we do not have appointments on weekends or after 4 pm.
PLEASE CAREFULLY READ THE PARAGRAPH BELOW:
You must first register and complete the required preregistration paperwork to request an appointment. Use the Calendar tab to choose your appointment. Your appointment request will be confirmed after your insurance is confirmed. To avoid delay, please upload a copy of your driver's license and the back and front of your insurance card. Once your appointment is confirmed, please fully complete your required assessment paperwork to avoid appointment cancellation (required for all patients; do not skip questions or omit answers; the assessment paperwork will take an average of 20 minutes to complete). Please note that the assessment paperwork (health and mental health history) is required because our clinician is a psychotherapist, making this information necessary.

VOICE MESSAGES - Current patients' high Distress/Crisis messages will be addressed within the hour. All other messages will be returned as soon as possible by the next business day. We encourage all patients to use the patient portal messaging system for faster response time.

HIGH DISTRESS/CRISIS SESSIONS for Current Patients Only
CANCELLATION/RESCHEDULING/NO-SHOW: When a NEW PATIENT cancels, reschedules, or is a no-show for their initial appointment, the next advance booking becomes restricted to no more than 2 days before the date they wish to receive services. Last-minute cancellations/rescheduling/no-show impacts the patient, the psychotherapist, and other patients who could have utilized that time slot. Sessions are scheduled in advance and reserved for you.
Current Patients may be responsible for the entire fee if a no-show or cancellation/reschedule is less than 72 hours (4 business days). This is necessary because a time commitment is made to the patient and is held exclusively for that patient.
PATIENTS WITH INSURANCE:
You are NOT responsible for the fees shown on the invoice if you have 100% coverage for rendered services. You will, however, be responsible for fees not covered by your insurance, for example: co-pay, deductible, shared cost, out-of-network, late cancel/reschedule, and no-show. Your insurance company will only pay a contracted rate for your services.
NO INSURANCE or NO REIMBURSEMENT BY INSURANCE COMPANY:
If you are responsible for the full payment of your session, you must submit payment before the start of your session.
If you are required to meet a deductible, co-insurance, or co-payment, you must submit payment before the start of your session.
